Daily Experiences Log
A Daily Experiences Log helps you track situations, thoughts, feelings, and actions. It helps you develop mindfulness skills like awareness, acceptance and defusion, encouraging reflection on whether actions were workable in the short and long term and supporting value-based decision-making.
Goal-Setting Worksheet
This worksheet helps you identify specific actions you’d take if therapy was working, focusing on meaningful changes across areas like relationships, health, and personal growth. Choose categories that matter most to you and reflect on actionable goals. Bring it completed or unfinished to your first session to guide our work.
Making Empowered Decisions Exercise
This exercise helps clarify your choices by examining the context, values, and workability of each option. It encourages decisions based on what matters most, helping you focus on acting in alignment with your values while being willing to experience discomfort.
Pre- and Post-Session Reflection Sheet
This optional reflection sheet provides structure to help you stay engaged and make the most of your therapy experience. It guides you in tracking progress, insights, and challenges, with space to reflect on key takeaways and barriers. Use it as a tool to deepen your focus and support your growth.
